Remember to complete forms in Skyward

Battle Ground Public Schools

All families of enrolled students are required to complete certain forms by Sept. 1. To get started, please log into Skyward Family Access.

Required forms

  • Annual student update: This gives you important options regarding your child’s education and ensures that we have correct contact information for you. We will use this information to communicate with you and reach you in case of an emergency. Once you are logged into Skyward, select the “2026-27 annual student update" tile. See instructions.

  • Health information collection form: This required form allows you to share information about your child’s health history.

Optional forms

Additional forms are available to give you options regarding your child(ren), including surveying, accessing materials through the Ft. Vancouver Regional Library and more.

Don't have access to a computer?

The Skyward mobile app (available in the Apple and Google stores) will allow you to complete the form. In addition, your child's school has a computer available in the office for you to use.
